Foster Care Case Management
Our Little Haven’s contracted Foster Care Case Management Unit provides services for children and families involved in the foster care system due to abuse and/or neglect. The children we serve range in age from newborn to 21 years of age. Case managers assist families in the goal of reunification, but if that is not possible, they work to secure a safe, loving, and permanent home for foster children.
Our Little Haven started the Foster Care Case Management program in 2000 when the State of Missouri began privatizing a portion of their foster care cases. In 2005, we joined with several collaborating child welfare agencies to form St. Louis PARTNERS and successfully bid on the current performance based contract. We have expanded our services and now help provide foster care case management services for children and families in the St. Louis City, St. Louis County, St. Charles, and Jefferson County.
Under this performance contract Our Little Haven's Foster Care Case Management staff and our St. Louis Partners assess the needs of children and families, arrange and provide appropriate services and move children into permanency within a specified period of time. Moving children to permanency means that we want to place children in a permanent home/family situation as soon as possible. This could involve a return to the biological family, a relative placement or an adoptive home. Staff work directly with the children and families in the community, with our network of referral/support agencies and the courts to coordinate plans of care that meet the outcome criteria of this contract. Staff is on call 24 hours per day, 7 days per week, 365 days per year to serve families and take new emergency referrals.
